Can't download attached data and can't access to files

I have a new problem… I try to access and download the attached files but

and when I try to download the data the zip file have 22 bytes and is empty, but the files has been uploaded.

Could you test the same survey form on a public server (either OCHA or the HHI server) to see if it behaves in the same way?

Thanks for your response, the form works fine on HHI server and the zip attachment is not empty.

Any idea of how solve it on Own Server?

So it is an issue with your self-hosted server.
