Cascading choices can only reference one choice list

Hello,
I am new in kobotoolbox, and i have been practicing to get better.
But now i have an issue that occur when i try to edit my xls form.
Note: i can preview the form, but i cant edit.
ISSUE: Error loading survey:
cascading choices can only reference one choice list

I appreciate any help to solve this issue.

There is a missing bracket in the formula as in the picture.

I think that i must buy some glasses…

Now i am able to deploy the form, but am still having the same error when i try to edit it.

As for the name column, it must not have a duplicate
I modified the choices sheet,
I guess the mistake won’t show up again.
