Colour Code of a Note Field

Hi Team,

Could you please help us on achieving the below criteria.

The form should consist of two fields:
One is integer
Second is Note.

Q1: Enter a number (interger)
Q2: Colour (Note field)

For the second Question we are having range conditions.
For Example:
If the user is entering 1 to 4 then the note field should display in Green Colour
if the user is entering 5 to 8 then the note field should display in Orange. Colour
if the user is entering 9 to 15 then the note field should display in Red. Colour.

Note: we are able to achieve in the format of text as shown in the below image but we want to display whole note as respective colour based on the range values.

Instead of gray we need to display Green. is it possible?

If possible, please provide us XLS file.
