How can I make left and right sided question and answer form?

We know KoBoToolbox works, up and down question and answer method. Always question stay on the top and the input method means the answer is always down. Is it possible to make side by side question and answer? (like the picture)

That means the Question will be on the left side and the answer input form (Value) will be on the right side.

I also want to make always show dot (.) in the input form. (Like the Picture) koboform%20input%20value
