I followed some feedback in the community and was able to identify some of the problems.
However, I couldn’t preview still without any error message.
Seems like you have multiple issues. To check the errors from your survey form, you need to download them as XLSForm and validate them against this online validator.
Regarding your second issue, once you have downloaded your form as XLSForm, you could add skip logic as needed to the variable you wish.
