Kindly please be informed that the screenshot of the form as shared can be designed in KoBoToolbox but may not see exactly as what is seen in the hard copy.
Since, it’s a household roster that you wish to design in KoBoToolbox, you could try out the repeat group feature as outlined in the support article here.
