I am designing a survey and need guidance on informed consent. I must (1) read a Participant Information Sheet (PIS) to respondents and (2) collect written consent using my organisation’s form before proceeding.
Could you advise how to: 1) Include the PIS in the form for enumerators to read 2) Ensure the survey only continues if consent is given 3) Capture written consent (e.g., signature)

I am using the app-based mobile version of Kobo.

Based on what you’re trying to achieve, I’ve put together a simple working example.

You can include the Participant Information Sheet (PIS) directly in the form using a note question type, or you can provide a hyperlink in the label column, for example:
Press [here](https://yourorg.org/full-pis) to read the **Full Participant Information Sheet**
To record consent, use the acknowledge question type, which is designed specifically for capturing acceptance.
