Import Error message

Hi am trying to create a new Project and when I try to import the XLSForm I get an error message:

ImportName: PDA PDM Duplicate name except

FailedSurvey. Duplicate node name

    Tool_2026.xlsOther_specify what could be the problem? PDA PDM Survey Tool 2026 is the file name
