Information from a list to be used during the survey

Hi all,
I have a list of items (Item ID, Description ID and Unit of measure). I will choose for instance the Item ID using “select one” function and I want to put the rest (Description ID and Unit of measure) in text fields (abc question) in the form. Please advice. Here is the sample of the list I will use. Many thanks.
