Jumbled Matrix questions

Hi, I am struglling to sort out matrix questions keeps having the rows jumbled… has anyone had similar issues and have how did you/they resolve this?

Welcome to the community! Would you mind trying it with a different project and see if the issue still repeats. If it happens to repeat again, would you mind sharing the server you are using (i.e. OCHA or HHI) and also a screenshot of the issue (with what it should be and what is not happening).
