hello guys kindly help, I am getting this error but i cant really undrstand it

Please ensure all names in your form are unique - you have several duplicates, not just the one above that is throwing the error

I also notice you have a couple of groups with the same name as questions (eg ‘choosing_telecomprovider’). Although this may work (?) it is probably asking for trouble down the line… Again, try to ensure all names are unique.
