Need help on Form designing

I want to design a Form for retail shops where I want to capture following information:

  1.   30 brands ( 1 to 30)

  2.   15 possible categories for each brand (1 to 15)

  3.   5 SKU’s (sku's for each Brand+category)

  4.   MRP of each SKU (mrp for each Brand+category+sku)

  5.   PTC of SKU’s (ptc for each Brand+category+sku)

Please advice as what logic should be best for it or provide reference if any.
