Piping In Name, Address and Email Address

Hello, I would like to Pipe in Name, Address and Email Address ([NAME] of [ADDRESS], [EMAIL ADDRESS])

Am trying to script a contract where the respondent types in their details so I would like to pipe them in as in the exerpt below:

This Contract is made on the …, by and between Organization, on the one hand and [NAME] of [ADDRESS], [EMAIL ADDRESS] herein after referred to as “the Consultant”. Organization and Consultant will all be referred to collectively as “Parties”.

How do I achieve this?
