The name column for the ‘oj6il40’ choice list contains these duplicates: ‘__’. Duplicate names will be impossible to identify in analysis unless a previous value in a cascading select differentiates them. If this is intentional, you can set the allow_choice_duplicates setting to ‘yes’. Learn more: XLSForm.org.

Often using the search function of this forum can provide solutions and examples, e.g.

But it is very rare that duplicates are wanted. So, you better make the item names per choice_list unique.

@bimarsh, in addition to what @wroos has suggested you could also validate your xlsform through this online validator. The online validator should be helpful for you to identify syntax issues present within your xlsform.
