Had a bit of a random one occur.. We have response dependent skip logic which is based on = response from select one question type. We build our forms through the builder on the website.
Where the skip logic was based on two or more different options (and had been since October 2025 last updated / redeployment occurred), the options ended up being duplicated. So what was once:
= Completed Session
= Final Complete Session To Close Client
Became
= Completed Session
= Completed Session
This happened across all the questions with these types of skip logics included in this survey. We returned the form to the it’s correct skip-logic setup, but also checked deployments and could see there’d been no updates or amendments recently to evidence the change.
Has anybody had anything similar happen recently? Wondering what’s going on…

FYI there was an issue (now fixed) whereby if you explicitly duplicated a select-one or select-multi questions in Formbuilder, they would end up basically sharing the same choice list underneath, even though the two questions were now otherwise completely independent. As a result, if you subsequently happened to edit the choice in one of the selects, it’d have the side-effect of (silently) changing the choices in the other.
This has recently been fixed (so if you now duplicate a select question, an entirely independent copy of the choices are made for it), but if you had previously duplicated a select question some time ago then they may still be ‘linked’ together in this way. Could this be causing the issues you are seeing? One way to confirm whether your two (or more) selects are still linked in your form would be to ‘Download XLS’ your form and then examine whether you have any select questions referencing the same list_name.
