[SOLVED] Critical Issue Affecting Multiple Projects: Submission Viewer, Image Attachments, and Enketo Functionality

Dear KoboToolbox Support Team,

I am experiencing a critical issue affecting multiple KoboToolbox projects on kf.kobotoolbox.org.

Our forms have been functioning normally for a long time, and we have successfully collected data through approximately 57 projects, many of which contain more than 500 submissions. The issue appeared suddenly without any recent form modifications, redeployments, or configuration changes.

Issue Description

The following problems are occurring across multiple projects:

  1. The Submission Record View is not displaying data correctly in some projects.
  2. When opening individual submissions, image questions do not display the associated photos.
  3. Instead of the image previews, only the image filenames are displayed.
  4. When attempting to download images from the submission view, the following error appears:

“Couldn’t download - No file”

  1. In several projects, I am unable to open or edit submissions correctly.
  2. The issue affects both old and newly submitted records.
  3. The problem occurs across multiple forms and projects, not just a single form.
  4. The same behavior occurs on different computers and in different browsers.
  5. Some projects that previously functioned normally are no longer displaying records and attachments as expected.

In addition, while attempting to edit records, I have also encountered Enketo-related errors such as:

“There was an error loading Enketo.”

What Is Still Working

Despite the issue:

  • Data remains visible in Data → Table View.
  • Photos remain visible in Data → Gallery.
  • Existing submissions are still present.
  • The total number of submitted records appears correct.

Troubleshooting Already Performed

I have already:

  • Tested using both Google Chrome and Microsoft Edge.
  • Tested on multiple computers.
  • Checked multiple projects and submissions.
  • Confirmed that photos are still visible in the Gallery section.
  • Verified that the issue affects both historical and recent submissions.
  • Created and tested additional forms to compare behavior.
  • Confirmed that the issue affects multiple users.

Additional Information

Because the records and photos remain visible in the Table and Gallery views, it appears that the data itself still exists. This suggests the issue may be related to:

  • Submission Viewer functionality
  • Attachment retrieval and image rendering
  • Enketo integration
  • Form metadata
  • Record editing services
  • Another platform-side component

Yes, we’ve identified an issue with viewing photos when editing submissions in Kobo, although the photos, etc are all still present in your project. A high priority ticket has been logged and the dev team are actively working on it.

A fix for this has been merged and tested, and should appear shortly on Global & EU servers in the next patch update. Thank you for your patience.

I am experiencing what seems to be the same issue reported in these topics:

In my case, images collected are no longer being displayed in the Enketo/submission view. Instead of the image preview, only the image filename is shown, for example:

I performed several checks:

  • The submissions were successfully sent from KoboCollect.
  • The image files are still stored on the KoboToolbox server.
  • Clicking the download icon downloads the correct image normally.
  • All images are displayed correctly in the Gallery view.
  • The image requests in the browser Network tab return HTTP 200 or 304, with no 403/404/500 errors.
  • I tested using an incognito window and the issue remained.
  • The problem is also affecting older submissions where the images were previously displayed correctly.
  • Multiple submissions appear to be affected.
  • We are using the official KoboToolbox servers, not a self-hosted installation.

The browser console only shows messages such as:

InstanceID and deprecatedID population disabled.
Preloaders disabled.
Calculations disabled.

There are no apparent image-loading errors.

Since the same images can be downloaded and are displayed normally in the Gallery, it seems that the media files themselves are intact and that the problem may be specifically related to the Enketo/submission view image rendering.

Could the KoboToolbox team please confirm whether this is a known issue or a recent regression?

This looks more like an Enketo rendering issue than a problem with the images themselves, since they download and appear correctly in Gallery. The fact that older submissions are affected too could indicate a recent regression. Hopefully the KoboToolbox team can confirm and address it.

I also believe it’s a bug. I hope they can fix it as soon as possible.

I’m having the same problem, when printing (.pdf), the pictures don’t appear while visualising the registry (it’s more clean than printing to pdf directly in the button).

I’m having a similar problem. I can’t export the file with the images. They are indexed, and I can see their file paths and download them, but they are not displayed when I try to export or print the file.

Por otra parte para usar imágenes dentro de los formularios creados con Kobotoolbox puedes revisar este post discutido anteriormente:

al momento de ingresar en el modo edicion de los formularios no se visualizan las imagenes, este problema lo vi hoy, ya que el dia de ayer estuvo sin problemas, y esto aplica a todos los formularios antiguos tambien.

Me dirijo a ustedes para reportar una serie de inconvenientes técnicos que estoy experimentando recientemente con el manejo de archivos multimedia (imágenes) en mis formularios. Estos problemas están afectando significativamente la calidad y eficiencia de nuestra recolección de datos.

A continuación, detallo los tres problemas específicos que he detectado:

1. Desaparición de la previsualización de imágenes antes del envío

Actualmente, al momento de querer enviar un formulario finalizado, ya no es posible previsualizar las imágenes que han sido cargadas. Anteriormente sí se mostraban, lo cual era fundamental para que el encuestador pudiera verificar que la fotografía adjunta era la correcta antes de sincronizar los datos.

2. Error de “Archivo no encontrado” durante los envíos

En algunos formularios, al momento de realizar el envío, el sistema no logra procesar la fotografía y me arroja un mensaje de error específico indicando que no encuentra el archivo.

Ejemplo del error mostrado: “No se pudo encontrar 57392-9_25_44.jpg”

Este problema provoca que los formularios se envíen sin la evidencia fotográfica correspondiente, a pesar de que la imagen fue tomada/cargada correctamente en su momento.

3. Rotación automática y errónea de las imágenes

He notado que algunas de las imágenes cargadas sufren un cambio de orientación automático e involuntario, rotando a formato horizontal. Esto altera la visualización correcta de la fotografía y dificulta la revisión de los datos en el servidor.

Agradecería mucho que su equipo técnico pudiera revisar estos incidentes.

We are also facing the same issue, and it is having a significant negative impact on our reporting system. We share Kobo reports with the client on a daily and weekly basis, and this problem is affecting our ability to maintain timely reporting.

We kindly request that this issue be resolved as soon as possible. If anyone is aware of the estimated time required to fix the problem, please let me know, as we are heavily dependent on the Kobo reporting system for our project monitoring and reporting activities.

  2. Respecto al tercer punto que reporté (las imágenes que rotan solas a formato horizontal), ¿este error también quedará corregido en dicha actualización?

  3. Mientras se implementa la nueva versión, ¿existe alguna solución temporal o configuración que nos recomienden aplicar para mitigar estos inconvenientes?

Thank you very much. The Kobo (There was an error loading Enketo has been) resolved. Now we can edit the form; the only remaining issue is that the photos are not being displayed.
