Well i observed the following issues within your xlsform:

In row 36 of your survey tab: space in name column, part_ two should be part_two
In row 24 of your survey tab: space in expression $ {last_mensus} should be ${last_mensus}
In row 25 of your survey tab: space in expression $ {last_mens} should be ${last_mens}

I made the changes and could upload without any issue. FYI:
