Hello @stephanealoo I came across this error, I have attempted to solve it by building a form in a new sheet but in vain, still receiving;
unable to deploy
your form cannot be deployed because it contains errors:
[row : 44] List name not in choices sheet: 12

Hi, This is an error on your form. You seem to have indicated a choice list that does not exist. I would recommend that you always check your form, resolve any issues, and recheck until you have no further issues. You can do this here
