Welcome back to the community, @nrmc! Have you validated your XLSForm through this online validator? The online validator should be able to help you identify syntax issues.

As a heads up please be informed that your XLSForm seems to have a lot of questions under the survey tab (5621 questions). This is a lot. Maybe you will need to break your questions to several forms as the system may timeout.
