It is generally a good idea to validate your form in online validator.
The online validator says:
Error: [row : 126] Invalid question name [d_10_ b] Names must begin with a letter, colon, or underscore. Subsequent characters can include numbers, dashes, and periods.
When I check your row 126, it seems you put a space between _ and b: d_10_ b
Changing that to: d_10_b should solve your problem.

problem is in the choices sheet, Instead of ${tmpf1_wpt40} use thislist_name
