Using Insert Cascading Select

I have 4 different provinces, with 8 different districts, with 117 different catchment areas and 578 different health facilities. I really need a step-by-step guide on how to cascade all these information in my in my KoboToolbox account using insert cascading select. Any tutorials or guide on how to do this is much appreciated. Thank you one and all.

Hello @Edze,
I have reviewed your file And Found more then 400 space in variable name. Please follow the Rules of Naming variables (remove the spaces in variable name), also use unique names.
