Welcome to the community, @jznakato! Maybe you could do the following to see the syntax issue within your form:

  • Download your project as XLSForm as outlined in this post discussed previously.
  • Open your XLSForm. Press CTRL+F and then paste qc52n59 in your survey tab as well as your choices tab to see if it’s missing in either one of them. If it’s missing in either one, fix it as needed.
  • Validate your XLSForm through this online validator to see if there are any other syntax issues.
