@theo27, I see two different approaches for you with this.
Approach 1: when you intend to collect your data offline
Share the Excel file with your enumerators/respondents before filling up the survey form. So here, they will need to fill up the details in Excel, upload the information as a file and submit it to the server when the enumerators/respondents are back online.
Approach 2: when you intend to collect your data online
With this approach, you could share your Excel as a google sheet while collecting data within the form.
