Images not displayed in survey form

I have used only one language. i.e. Nepali. I have tried by considering different methods

  1. only label (no language coding)
  2. label::nepali and also use media::image::nepali
  3. follow Adding images to questions - #2 by Kal_Lam process
    but not opening. could the form is same which I have sent and image as per above attached.
