I would like to know if it is possible to create a form with pre-existing data. I have in an excel table the following information:
1- The region;
2- The department;
3- The sub-prefecture;
4- The village / Locality;
5- The name and first name of the beneficiary;
6- his gender;
7- the number of people in the household over 70 years of age;
8- the number of disabled people;
9- the neighborhood of residence;
10- The beneficiary’s contact;
11- The name of the area supervisor;
I would like to create a form with this pre-existing information, and complete with new information which are:
1- The number of girls from 0 to 5 years old in the household;
2- The number of boys from 0 to 5 years old in the household;
3- The number of girls from 6 to 18 years old in the household;
4- The number of boys from 6 to 18 years old in the household;
5- The number of pregnant women in the household;
6- The number of breastfeeding women in the household;
7- The number of men in the household;
8- The number of the identity document of the beneficiary or his representative;
9- Finally, possible, the signature of the beneficiary.
I would like to attach a copy of my table so that you can better understand my concern.
Also, is it possible to have the procedure, but above all a model on which I could base myself to build my survey form?
