@testwbd, when I tried validating your XLSForm through this online validator I see this error message:
I then quickly scanned your XLSForm (the survey sheet) and did not find any name ${Variety} under the name column header. But I could see that you have used it frequently under the label column header.
This is the reason why you are not able to deploy your survey form.
