Hi,
The easiest way to add your own logo to your forms is to upload a file called form_logo.png in the “Existing Form Files” section of your Project’s settings.

One convenient option might be to use the Enketo web form version of your form, then create a shortcut to that form on your homescreen. This way it will look like other apps, including the title of your form, and opening it will show your logo along with the content of your form - nothing else.
To rebuild the app and rebrand it you’d need to have solid experience developing in Java; the code is open source so you can change whatever you like, including the title.
