Add group headings to downloaded file

I have the following form.

I have a group named details, with various fields in it. When I download the form data, the file only shows the heading of the fields, it does not mention the name of the group. This is actually very important because this form contains a lot of diseases that are divided into groups, it will be a big problem if the group name is not included in the file. Is there a way to accomplish this. For example:
Details
month date no of working days district

I don’t think so, you might need to use some excel to achieve that.
