An error occurred when trying to open the data entry form

I have created a new form in which unique beneficiaries are entered, in accordance with the instructions from the discussion:

Those colleagues who created accounts a year ago, including me, use the form without any problems.
However, colleagues who created accounts a few days ago (after KOBO moved to a new server) receive an error when trying to fill out the form.
What could be the reason and how to fix it?
