Hello, I can’t seem to figure out how to assign numeric values (0-4) to each of the answers on a multiple-choice question? Any help would be greatly appreciated. Thanks.

Thank you for your welcome and your prompt response. I’m trying to create a prediabetes risk assessment for a free clinic. Attach is a picture. I would like to assign a numeric value 0 to less than 40 years old, 1 to 40-49 years old, 2 to 50-59 years old, 3 to 60 year or older. Then I would like to assign numeric value 0 for man, 1 for woman. My intent is to calculate the numeric value at the end of the assessment to determine if the patient is at high risk for diabetes.
This can be easily done by clicking on the fields you have highlighted and entering the number you want. It’s that simple. I have been doing this all the time.
