i tried it and i receive that “Error: [row : 181] On the ‘choices’ sheet, the ‘name’ value is invalid. Choice names must be unique for each choice list. If this is intentional, use the setting ‘allow_choice_duplicates’. Learn more: XLSForm.org

@akadio1, it should be a form design issue. You should be able to learn more about the same through this post discussed previously:

Hi @maggib, I found the duplicate that i have highlighted with yellow and marked the font with red within list_name lf3sx85 whose name prefer_not_to_answer is seen repeated twice within list_name lf3sx85. For your ease check row number 89 and 93 under choices tab. All you need to do it go to your choice sheet and then check row 181 to check for duplicates in your choices sheet.
