To solve this issue go to the setting sheet in your xlsform and do the following:
1- In the header put this statement “allow_choice_duplicate”
2- In the cell below put “yes”
3- Save your file and upload it.

No need to create a new form. This is an update for your form and you will be able to receive the submissions in the same form. But you have to do it correctly.

Hi @anna_sda, When i tried to validate your xlsform through the site here i could see some issue (as outlined below): [image] When looking at the choices tab of your xlsform i see that row number 381 and 386 has the same name 2 assigned. Here is the list of duplicates that i have highlighted in your xlsform. Try giving them a unique name and the problem should be solved.

Note: But at the same time also please be cautious that if you change the name in the choices tab, you should also change the same in the survey tab as there may be some association with skipping and constraints.
