When i tried to validate your xlsform through the site here i could see some issue (as outlined below):
When looking at the choices tab of your xlsform i see that row number 381 and 386 has the same name 2 assigned. Change one of them and your issue should be solved.
PLEASE ENSURE THE SAME WITH OTHER CHOICES TOO.
