You seem to have included spaces and other non-accepted characters in your name column of the choices. Could you confirm what error message are you getting? Send a screenshot of the same.

Hi Milon,

I have worked on your shared XLSX form and created a Uploadable File which works. Some Upazila > Union may not be accurate, Otherwise, the form is Okay.

Let me know, if you need any further support in this regard. I tried to upload the form here but I could not. It says that I am new to this forum, thus I can not upload any file.
