This is called a matrix question. Please note that it only works in Enketo (Web Forms). You can find more information in here:

Noted.Though I tried to design a form to suit what I want, but I have failed to develop it.

@Faith, please note that you may not get the exact structure of what is there in the hard copy but you should be able to get a similar table with KoBoToolbox.
