Creating serial code

I do have a request, I wanna create a serial code starts with ICLA-Krk-0001 and so on; would you please advice on how to do this?

Hint: I have attached the xls form; can you please add it to the XLS form as I need it very quickly

I don’t want the data collectors to type the ICLA-Krk-
as this is time consuming in the field
