started COVID Sahayata Kendras (CSK) or
COVID Facilitation Centres in Kolkata and rural areas of West Bengal including tea gardens in
North Bengal in partnership with local health authorities. One in Jharkhand and others are
planned in Odisha and Assam. At each centre trained volunteers provide counselling to remove
fear and anxiety, get people to wear mask, hand wash frequently and observe social distancing.
Wellness Kits containing masks, soap, sanitary pads and Nutrimix, a nutrition booster for children,
pregnant and lactating mothers, are distributed to families in need. So we need to customized form design , in this regards we want to create master table and other relational table to store data.
