Dataset shows two columns for every question & “record name or id already exists” on submit

• I removed some groups from my form.

• In the downloaded XLSForm’s survey sheet, each question name appears only once.

• Yet in Data → Table on Kobo, every field shows up twice.
  

  

• Submissions now fail with “this record name or id already exist please change it”

Question

How can I eliminate those duplicate columns and fix the record-name error—while keeping the same form URL?
