There should be a choices sheet in this xlsform. When looking for a sheet named ‘choices’, the following sheets with similar names were found: 'choices '. Please ensure that the choices sheet has the mandatory columns ‘list_name’, ‘name’, and ‘label’.
Welcome to the community, @abdisalam1100! It seems like you have a select_one or select_multiple question type in your survey tab, but you have not defined the codes in the choices tab. You should be able to learn more about the choices tab through this site. As a backup, you should always be able to validate your XLSForm through this online validator.
