I made some mistakes and it took me a while to amend, so please help me to overcome a solution for this below message, although i wanted to attach the form but is saying new user can’t upload
There should be a choices sheet in this xlsform. When looking for a sheet named ‘choices’, the following sheets with similar names were found: 'choices '. Please ensure that the choices sheet has the mandatory columns ‘list_name’, ‘name’, and ‘label’.
