I tried to develop survey form with drop down questions. fist of my two questions are working fine but the third is not. Please suggest.
It seems that your cascading choice list combinations are not complete. Every “parent” must have at least one related “child”.

@yogendrakatewa99, you should be able to design a cascading select question as outlined in this support article Adding Cascading Select Questions.
