My form is can entry via browser but cannot download result failure in mobile. Please help me.

Could you share with us the configuration of your Collect android app from the General Settings>Server?
In the Collect android app’s General Settings>Server>URL:

(enter the password that you use while login to your user account drcactivitymis)
Note: You are not able to get blank forms for a shared project if you follow the method you have shared above. However, if you follow the one that i have shared here it should solve all your issue whether it’s your own survey project or it’s a survey project shared by someone.
