Email notifications to notify the supervisor once a submission has been made

On the same issue, I have configured email notifications to notify the supervisor once a submission has been made. My issue is on email it does not show the names of the user who has submitted on the email body. Example;

I have configured this using flows as below;

And in the Rest Services as below;

What I missing on the configuration to ensure that name and emp number appear on the submitted email body? This would really help in our operations.

What is the variable name that you have used in your data collection form that should be notified to your supervisor?
