These are only warnings, not causing your problem. Using groups without label is a design option.
Side-notes:
- None of your questions/variables in your form is mandatory. So, totally empty forms can be submitted.
- Never post person-related data, like names, phones etc. to the forum, please. e.g. facilitator names in your attachment. See community Guidelines.
