OK. Would you mind validating your question to see if it contains any skips or if it does not have a mandatory response? Generally, these are the two things that will affect the completeness of your survey response.

In addition to what @Kal_Lam has indicated, i would recommend that you also check if you have a previous deployment of the form where these questions were missing. If that is the case, then your issue is that there are blank.
