Forms unable to deploy

When trying to deploy forms i am getting an error:

I think there is an issue with your form. Can you check on the highlighted and indicate what you really wanted to achieve with them.

Try this out, it should work. Did nothing (but simply changed the variables and it worked)…

In addition to what @Kal_Lam has indicated, I would recommend that you add the relevant criteria highlighted if you intend to achieve the following.

This questions shhould pop up only if the question "why did the refrigerator fail is answered.

The logic being that the condition that determines the response to why did the refrigerator fail should apply to the rest of the questions as shown below
