"Get blank form" isn't showing up on Kobo Collect

Hello community,
I am new to Kobo and I followed the configuration on the support link.
I could create my form and deployed it:

I then set Kobo Collect up according to the guidelines on the support website:

URL: https://kc.kobotoolbox.org
Username: xxxxxxx
Password: ****** (the correct one)

But no matter what I try, the “Get blank form” option isn’t available.

Could you please help me? Thanks

The “Get blank form” isn’t showing, meaning the server hasn’t been linked.

It has been replaced with “+Start new form”.
