How to access a form created and deployed in for example kf.kobo-example.ga

I am having a challenge accessing data collection form I created and deployed in Kobo Private Server (e.g. https://kf.kobo-example.ga).

I am used to getting forms using the https://kc.humanitarianresponse.info/ but have never accessed forms using the private server settings

Any assistance how this works will greatly be appreciated
