How to add a heading to a rating question

Hi all,

I’m tasked with creating a rating question similar to this:

I need to have the “Source of Stress” and “Source of Support” headings to separate the options, but I don’t see any way in the form builder to do so.

Any advice? I did check the documentation but didn’t see what I’m looking for.
