You want to have a text question that requires the response to be UPPERCASE ONLY? There’s no way to display only an uppercase keyboard, but you can use validation criteria to reject responses that have lowercase characters.

You might also be interested in using the translate() function, which could change lowercase characters in a response to uppercase characters: Form Operators and Functions - ODK Docs
